CMS has awarded $8.7 million to a New York-based not-for-profit to leverage health IT in an effort to improve HIV/AIDS medication adherence, Health Data Management reports.

VillageCare, a provider of post-acute and chronic care services, received the grant as part of CMS’ Center for Medicare and Medicaid Innovation’s second round of Health Care Innovation Awards.
Under the three-year project, called VillageCare Wellness Innovations, an integrated mobile application and Web portal will serve as a mobile medication planner. The platform will provide services, such as:
• Personalized medication adherence plans;
• Peer mentoring;
• Social networking;
• Text reminders;
• Virtual support groups; and
• Virtual access to health coaches.
According to Health Data Management, VillageCare will recruit 5,000 patients for the project, which is set to launch in April.
The mobile platform was created by Wellness Layers, a patient engagement services provider (Slabodkin, Health Data Management, 3/24).
